New brachytherapy treatment planning system was developed using 3D-Slicer as front-end software. The proposed MC-PRISM (MonteCarlo based Prostate bRachytherapy plannIng System) was wrapped into 3D-Slicer. MC-PRISM allows fast Monte Carlo (MC) simulation with local or distant calculation on server. The dose provides by MC-PRISM is fully personalized according US and CT images of the patient. MC-PRISM is used to automatically perform the inverse planning for whole gland, hemi-gland, boost and focused prostate brachytherapy. The resulted plan do not need to be tune by dosimetrist thanks to the DVH-based optimization and the cost function based on the AAPM TG-137 dosimetric criteria. Finally, this new TPS allows a complete MC-based inverse planning in less than 1 min using a standard CPU (Intel Core i7 with 4 cores).
